Hey bsor79! Sorry to hear about the problem with your pleco.
I am sorry to say that this sounds like a possible case of Septicemia, which is an internal bacterial infection which affects the nervous system of fish...and other living creatures such as humans - it is actually the 'technical' term for blood poisoning. While I admit that I wouldn't place 100% of my faith in the resource that I found this on, but I have read that the bacteria which is usually connected to Septicemia is gram-negative - which could be a good thing to note when finding a medication to use for treatment - just be sure that, if you are going to use medication (probably an anti-biotic obviously), it is of a variety in which you are adding medication to your fish's food rather than treating by adding medication to the water. It is also advisable to somewhat 'starve' the fish which you are treating, but only to help ensure that the fish is hungry enough to eat the medicated food as quickly as possible since the medication will 'leach' out rather quickly. In the case of a pleco, I would also think that using a quarantine tank would be very beneficial since this way you would have much more ability to observe the fish through strategic decorating (I would give some sort of hiding place for a pleco, regardless).
Please understand that my comments above really cannot be described as much more than a guess as to what might be the cause of the twitching you have noticed with your fish and so I admit, I could be completely wrong - hence, I suggest taking the idea of Septicemia a bit further by performing some more in-depth research and observations....which I might do in a hurry because if Septicemia is the problem, your fish could be in dire straits already.
